Grants

AlohaCare supports community-based innovations that strive to improve access to quality health care for low income and underserved populations. Annually, Imua Loa grants are approved on a rolling basis, rolling basis until funds have been exhausted for the year. Proposals must include a description of the project goal(s), timeline, target population, progress measures and deliverables, project team and organizational partners. Awardees are required to report outcomes at least annually.

Application Process

STEP 1: Submit Form

Applicant submits complete online application, including an IRS W-9 Form.

STEP 2: Application Review

AlohaCare reviews application for completeness.

STEP 3: Follow Up

AlohaCare may request additional information and/or a meeting.

STEP 4: Decision Notice

AlohaCare will provide written notification via email of the grant status decision.

STEP 5: Establish Grant Term

Letter of Agreement is sent to signatory. Once signed, AlohaCare CEO adds signature and agreement is considered executed.

STEP 6: Grant Funds Distribution

Funds will be disbursed based on agreement.
